Four Corners CHAP is the result of a collaboration between the City of Farmington’s Community Relations Commission and the Farmington Police Department. There are numerous services and nonprofit organizations dedicated to assisting the homeless and substance abusers living on the street, and CHAP seeks to help improve communication and resource coordination among these services. Organizations are invited to use this site to exchange information on the services they’re providing, and individuals in need of services will find valuable information on help that’s available in our area. CHAP meetings are held every other month, bringing together the various services to exchange information in person. Please use this site’s forum to ask and answer questions, and share information.

Hi Leigh,
After speaking with several homeless clients just recently, it was conveyed that a large barrier/obstacle for many of them (some out of state) was obtaining federal and/or state ID. Some did not know where nor how to start the ID process, others did not have the financial resources for postage and /or fees needed for this documentation. Do you think this matter would be a conversation for our next meeting, if it has not already been addressed. Thanks, Lynn
Lynn, I think that would be a great topic. Al Garcia is planning the August CHAP, and I’ll pass your comment on to him. I think we could find some answers for this problem (I know Workforce Connection helps with i.d.s, and we could get in touch with them, among other people). Thanks for the suggestion – Leigh
I am interested in your group was wondering, how I could get more info about you guys…I try to do as much as I can to help the homeless. But I feel I can do more…..the ladies at the Indian shelter told me about you….so I thought I would check it out…..Darren Lobato
Darren, we hold our CHAP meetings usually the second to last Thursday of each month at 1:30 at the Civic Center.
