Four Corners CHAP seeks to bring together the various agencies and organizations in the Four Corners area that provide services to the homeless and those living on the streets due to substance abuse. The primary mission of CHAP is to facilitate information-sharing and coordination of resources among these services and organizations.

Four Corners CHAP is the result of a collaboration between the City of Farmington’s Community Relations Commission and the Farmington Police Department.
